Details :
A thin electroplated disc made with copper. The disc has a flat rim on the edge, with a beveled rim just inside. Next to the beveled rim is a full wreath, made with leaves and berries. In the center of the disc is a raised, three-dimensional bust showing the head and naked neck of John W. Simons, facing left. Written on the base of the neck, in all capital lettering is "Lebrethon."

Written in a semi-circle in all capital lettering is, "John W. Simons" along the left side of the center and, "Gr. Master of Masons." along the right side of the center. At the bottom is written the date, "1861." Beneath the date is written, "S.H. Black, Electrotyper."
Th e reverse is coated with a dull substance.
